I recently got a job at a medical office to help out with data transcriptions and record keeping.  I assumed that, because the office was fairly new and the staff was fairly young, the office would be ultra modern and up to date in the latest technology.  I was unbelievably wrong.  As each patient comes into the office there are five or six pages of paper work, then add to those pages the notes of the doctor and his directions for the patient.  Then upon leaving the patient had two more pages to fill out and make a follow up appointment.  This office really needs some modern electronic medical records software.
